About the Role: We are seeking a Technical Lead – Healthcare Integration with 7–10 years of experience (or equivalent depth) to lead the end-to-end delivery of CDS- and EMR-focused integrations. This role builds on strong hands-on engineering and integration expertise and adds ownership of technical roadmap, execution, and technical leadership.
You will drive strategy and delivery across FHIR, CDS Hooks, SMART on FHIR, and legacy interoperability standards (HL7 v2, HL7 v3/CDA, X12), while partnering cross-functionally with product, program, data science, customer success, and external EHR teams. The ideal candidate is a player-coach—equally comfortable defining architecture and writing code, mentoring engineers, and ensuring operational excellence of a production platform.

Job Responsibilities:
Technical Leadership & Roadmap Ownership
- Own the technical roadmap and execution for CDS and EMR integrations, aligning with product strategy and customer commitments
- Define quarterly and long-term architecture and delivery plans, including prioritization, sequencing, and dependency management
- Establish engineering standards, best practices, and reference architectures for interoperability solutions
- Drive technical decision-making and trade-off analysis across performance, scalability, security, and maintainability
End-to-End Delivery Ownership
- Lead the full lifecycle of integrations:
- Discovery & Requirements (clinical, workflow, data, and regulatory requirements)
- Architecture & Design (API contracts, data models, integration patterns)
- Development Management (task breakdown, estimation, sprint planning, code reviews)
- Certification & Validation (EHR certification, CDS Hooks validation, payer requirements)
- Implementation & Go-Live (deployment, cutover, monitoring)
- Reporting & Optimization (KPIs, usage analytics, clinical impact)
- Ensure delivery predictability, quality, and adherence to timelines
Integration Architecture & Development
- Design and implement scalable integrations using:
- FHIR (R4+) APIs, profiles, extensions, and implementation guides
- CDS Hooks services and clinical decision workflows
- SMART on FHIR authentication and app launch patterns
- Support and modernize legacy integrations using:
- HL7 v2 (ADT, ORM, ORU, SIU, etc.)
- HL7 v3 / CDA
- X12 transactions (270/271, 276/277, 834, 835, etc.)
- Define patterns for synchronous, asynchronous, and event-driven integrations
- Contribute hands-on to backend and (as needed) frontend development
Cross-Functional & External Collaboration
- Act as the primary technical interface with:
- Product management (requirements, prioritization)
- Engineering (platform alignment, shared services)
- Program/Delivery (timelines, risks, stakeholder updates)
- Customer success (implementation support, escalations)
- Data science (CDS logic, models, evaluation)
- External EHR/vendor teams (Epic, Cerner, Athena, eCW, Meditech, etc)
- Lead technical discussions with client IT and clinical stakeholders
- Translate complex clinical and business needs into clear technical solutions
Platform Health & Operations
- Own platform reliability, scalability, and operational excellence
- Define and monitor SLAs/SLOs, observability (logging, metrics, tracing), and alerting
- Lead incident response, root cause analysis, and continuous improvement
- Ensure security, compliance, and data governance best practices (HIPAA, ONC, CMS)
- Drive cost optimization and performance tuning across cloud infrastructure
Data & Cloud Engineering
- Design and manage data models and persistence using PostgreSQL
- Optimize data pipelines and queries for performance and reliability
- Leverage AWS services (Lambda, API Gateway, SQS/SNS, S3, RDS, ECS/EKS) for cloud-native architectures
- Define CI/CD pipelines and release management strategies
Team Leadership & Mentorship
- Mentor and grow engineers through coaching, code reviews, and career development
- Provide technical guidance and unblock team members
- Foster a culture of ownership, quality, and continuous improvement
- Contribute to hiring, onboarding, and team scaling
Required Qualifications:
- 7–10 years of experience in healthcare integration and/or software engineering
- Proven experience leading technical design and delivery of complex integration projects
- Strong experience with PostgreSQL and relational data modeling
- Experience with AWS cloud and serverless/microservice architectures
- Familiarity with Ruby on Rails or similar backend frameworks
- Exposure to frontend frameworks (React, Vue, or similar)
- Demonstrated ability to own roadmap, execution, and cross-functional alignment
- Strong understanding of healthcare workflows, terminology, and interoperability concepts
- Experience writing and reviewing healthcare technical specifications and implementation guides
Preferred / Nice-to-Have Qualifications
- Experience integrating with major EHRs (Epic, Cerner, Athena, eCW, Meditech)
- Deep hands-on experience with FHIR (R4 preferred) and RESTful API design
- Practical experience with CDS Hooks and clinical decision support systems
- Strong working knowledge of HL7 v2, HL7 v3/CDA, and X12 standards
- Experience with event-driven architectures and message queues
